Eligibility for home health services varies by insurance coverage. Generally, you may qualify to receive home health services if:

• You require intermittent skilled medical care
• Leaving your home requires a considerable and taxing effort (e.g. you rely on the assistance of another individual or equipment such as a cane)
• you are under the care of a physician ordering skilled nursing or therapy

Visits from our care team are delivered intermittently and scheduled according to the patient’s needs. Home Health care services are designed to promote wellness and help you to regain independence.  Generally visits are most frequent in the beginning then taper down as the patient regains health.

Palliative care is specialized medical care, focused on pain relief and symptom management, for people living with a serious illness.  It may be introduced at any stage of an illness to provide comfort, improve quality of life and add an extra layer of support for the patient and family.

Hospice is a form of palliative care which means it focuses on patient comfort and quality of life. Most of our patients are cared for in the comfort of their homes or in a home-like setting of choice such as a nursing home, residential care home, or assisted living residence. If a symptom is difficult to manage in the home, a patient may be admitted to a hospital for short-term symptom management; however, it is the goal of hospice to help patients remain at home, in their familiar surroundings, when possible.

No, hospice care is for all ages. Hospice serves patients living with a number of different illnesses including, but not limited to, cancer, Alzheimer’s/dementia, renal disease, liver disease, AIDS, heart disease, COPD, and ALS (Lou Gehrig’s disease). Hospice care is available for patients who have been diagnosed with a life-limiting illness, regardless of the type of illness.

No. Hospice care is designed to supplement the care already being provided by a primary caregiver through regular visits and education from the hospice care team.  Our team will provide hands-on care during our visits, but we will also spend time to train the patient or primary caregiver as well as facilitate additional support through community resources where available.

In between visits, we are a phone call away (24/7) to answer ANY question or respond to urgent situations that may arise.

Hospice is 100% covered by Medicare, Medi-Cal and the VA.  Most private insurances also have a hospice benefit.  Bonita Springs is committed to providing care regardless of ability to pay. Yes; Residential Care Homes are licensed as Residential Care Facilities for the Elderly (RCFE) and must meet care and safety standards set by the State.  They are licensed and inspected by the Department of Social Services, Community Care Licensing Division.
